Recently I have encountered several patients who had unsuspected hernias found during their abdominoplasty. While this is really not that unusual, it is generally surprising to the patient. What is a hernia? Basically it is a "hole" in the abdominal wall, through which either fat or intestine can "poke through". It is usually not much of a problem but can become a medical emergency if whatever "pokes out" gets caught and can't get back in. This can cause pain and swelling. If severe it can require emergency surgery, performed by general surgeons. The most frequent place I find hernias are beneath the umbilicus (belly button). This is a naturally weak area because of the umbilical cord which nourished us as a fetus.
In any event during an abdominoplasty the skin and fat are lifted off the abdominal wall and the hernias come in to view. They are usually easily repaired and in fact the repair is reinforced by the usual muscle tightening which is done. Usually the hernias can be felt prior to surgery, but sometimes, like earlier this week, I found some in two different patients. Hernia repair itself is actually medically necessary and that portion of the surgery is covered by insurance plans.

While there are many different types of facelift procedures, the Composite facelift still provides for a natural balanced improvement of the aging face. After learning the technique first hand under the guidance of Dr. Sam Hamra, it has been modified slightly through the years to allow predictable beautiful results. Combined with a mid-face plication lift it is my preferred method of facial rejuvenation. The Composite lift retains the volume in the face and moves it back to where it belongs. Improvements in the technique have reduced recovery so that patients can "get back out" faster than ever. Let me know what you think.

Injectable fillers such as Restylane, perlane, Juvederm and Radiesse can be used successfully in correcting irregularities of the nose. Small depressions and irregularities of the nasal tip, bridge or even the side of the nose can all be "filled" with these products. They are particularly useful in patients that have had multiples prior operations. In these patients I would like to achieve improvement while reducing risk. Often these patients have had their stores of cartilage depleted and it can be "a long run for a short slide". As long as the residual deformity is small and not of any real functional significance the fillers work extremely well. It takes just  a few minutes in the office with essentially no recovery.

I received an email about the 15 minute nonsurgical nose job. Â email. The "non-surgical" nose job is performed by using fillers to hide imperfections or camouflage humps or bumps. It is true that at can take only 15 min and is very easy to do. Recover is minimal and you can return to full activity immediately though I recommend applying ice for the remainder of the day. The fillers used are the same as those used for wrinkles or depressions treated elsewhere on the face, such as Perlane, Juvederm, and Radiesse. Someone recently sent me an email asking about PTS sutures. They were reading online about these "progressive tension sutures". This is the thing.........
Progressive tension sutures are sutures (stitches) which are used to do two things. The main thing they are used for is to reduce the chance of getting a seroma after surgery. Seromas are unwanted collections of fluid beneath the skin. In general, excess fluid occurs in the body for a number of different reasons. This excess fluid is returned to the circulation through the lymphatic system. After abdominoplasty the lymphatics are temporarily disconnected, making it more difficult fot the fluid to leave the area. It's important to get rid of the fluid early so that the "skin" can "stick"back down. When the fluid collects it is called a seroma. Small seromas usually resolve by themselves slowly over a few weeks. Some however are larger and won't go away on their own. If they persist they cause swelling and prevent you from being as flat as you could be. Also there is a risk of having a seroma become infected, which is a whole different problem you want to avoid.
Anyway, these progressive tension sutures are placed to help fix the skin in position. You can think of them as a quilting type of stitch. When properly placed you should not see them at all. I use dissolvable sutures so that they go away after about six weeks anyway. Together with the use of drains, it is the best way to prevent seromas and improve your recovery. Oh what a difference a day in the operating room can make. For patients who have had large weight loss from either voluntary control or from bariatric weight loss surgery, body contour surgery makes the final transformation. Most patients who lose more than 50 lbs., not to mention those who lose in excess of 100 lbs., develop loose hanging skin. While this can sometimes be hidden beneath clothing, it is difficult to conceal and can be a real barrier from enjoying a more active lifestyle. Body contour surgery removes the unwanted skin folds and reshapes the body into more normal curves. The plastic surgeon becomes a body tailor. Generally the transformation is done in stages, beginning with an abdominoplasty and lower body lift. Next an inner thigh lift can be combined with breast surgery. The breasts typically sag and have lost volume. Treatment can range from a simple breast lift (mastopexy) or can include breast implants (augmentation) as well. Arm surgery, usually liposuction in combination of an arm lift (brachioplasty) can usually be done at the same time. Finally, attention is turned toward the face where facelift surgery can  be performed together with fat grafting to restore the lost volume and tighten the sagging skin. While these are the most common procedures there are others that also address different areas of the body. This type of surgery is typically performed in the hospital setting especially when combining several procedures at once. Patients who have had this type of body contour surgery are some of the most grateful patients I see. They have been able to experience first hand the power of plastic surgery in changing lives.

The recession may be officially over but everyone is still concerned about their finances. Financing has become a very popular way to allow patients to get the cosmetic procedures they want with an affordable payment plan. Wow. That means a lot to me. Â Breast augmentation is much more than getting "big boobs". In fact most of my patients are really just looking to feel more in proportion and want to remain "natural". Ok - well maybe natural with a little pizazz - but not the "Dolly Parton" look. In order to get the right look I ask patients to bring in photos of their "likes" and "dislikes". We go over them together during the consultation and discuss whether or not they are realistic goals and how best to get there. I take time with my patients so that we can decide on the right choices for that patient. Saline breast implants or silicone gel implants (gummy bears). Under the muscle or above the muscle. Round breast implants or teardrop breast implants. These are some of the things we talk about. In the end it's delivering the result that counts and receiving compliments like the one from CS tell me that I'm doing it right. Thanks CS.

Sculptra has recently been approved by the FDA for use as a cosmetic filler. It has been approved for many years for the use as a volume correction in the treatment of HIV patients. It has a long track record of being safe and effective. Like other fillers such as Restylane, Perlane, Juvederm, and Radiesse, it is performed in the office as a short procedure. It is used primarily to restore the volume that has been lost during the aging process. It is not intended to be used for the treatment of fine lines. It is different than the other fillers however in that it requires special injection techniques in order to deliver the best results. Unlike the other fillers, it is a more gradual process and actually takes 2 to 4 treatment sessions to complete the process. The big advantage of Sculptra is that it lasts for over 2 years! I have been using Sculptra for about 4 years and have found it to be an excellent choice to help give a natural correction to the aging face.

I recently saw a young woman, 17 to be exact, for consultation regarding cosmetic surgery. Â I was struck with the power of plastic surgery and the sense of responsibility to use it wisely. Through the years I have performed various cosmetic procedures on teenagers and have found it to be emotionally rewarding experience.The most common procedures include breast reduction, breast augmentation, rhinoplasty, otoplasty and occasional liposuction. Mind you, Baltimore is not like LA or Miami, where I am told that teenage plastic surgery "goes wild". On the other hand, Baltimore has teenagers that face all of the challenges found in any other large city. The pressures of fitting in and feeling part of the norm are probably never felt more acutely than during teenage years. We are living in a fast paced world where if you are not moving forward witht the group you are falling behind. For better or worse judgements are sometimes made on appearance and our bombardment from the media not so subliminally tells us what is beautiful and what is not.
On the surface, plastic surgery performed on teenagers may seem like simply caving in to the times. Not only is it "unnecessary" but perhaps "unnatural". I would argue  the opposite.  Under the right circumstances, plastic surgery on teenagers can be a most liberating and life changing experience on a person. Most of these patients have spent several years with either physical or emotional ailments directly related to their concern. The patients I see are keenly aware of what is "normal" and why they are "not". They are uncomfortable in their own skin and are searching simply to be "normal". Kudos to their parent(s) who have the courage to go outside of their own comfort zone to support and help their children.
So what does this have to do with me? Simply that I enjoy watching these "children" blossom after the weight has been removed. With the risk of sounding melodramatic, there is no bigger reward than to watch a person of any age come out of their shell and become productive, self confident and happy. Having that person be a teenager is no exception. Plastic surgery is certainly not a magic bullet that "cures all that ails ye" but it is a powerful tool that can do wonders.

I was speaking owith a patient today in preparation for her breast augmentation. She complimented me on her experience and my willingness to take the time to explain things to her. I am very appreciative and it got me thinking - isn't that the way it's supposed to be? Shouldn't patients expect to be able to see their doctor for more than 5 minutes, especially if they are trusting them with their surgery? And after the surgery is performed shouldn't they see the same doctor for their aftercare? It seems lately that there has been too much emphasis on fast procedures, fast money, and impersonal care. I can only tell you - that's just simply not my style. Perhaps it was watching my father, an orthopaedic surgeon, the way he would treat people, or maybe it's just the old saying "treat those as you would want to be treated", I find great satisfaction in being able to "help" people with their decisions, not just provide them with an operation. There's a lot that goes into choosing the right operation and the right surgeon and I think the process can be equally as important as the final solution. Anyway - perhaps I'll write more on this later. In the meantime - feel free to ask questions.

I read an article on Yahoo about muffin tops - that extra "bulge" which lies just over the waistband of your jeans. This is exactly what can be helped with an extended tummy tuck. Traditional, or standard, tummy tucks (abdominoplasty) addresses the front of the abdomen. It leaves behind the sides. Today's better techniques allows me to reshape your body all the way around. Surgery on the nose, or rhinoplasty, has always been a rather high profile procedure since the results are front and center. Unfortunately it is often the bad results on celebrities that get the most attention. What is overlooked is the tremendous satisfaction patients get once their nose "gets fixed". Whether it is the bump on the bridge , or the big round ball on the tip, patients want their nose to be less conspicuous and be more natural with fitting the rest of their face. With all of the emphasis these days on all of the fillers it may be that placement of cheek implants may really be the best choice. For those men and women who "never really had" cheeks to those who have "lost them" through the aging process cheek implants could be the perfect solution. The surgery for cheek augmentation is a short outpatient procedure that will restore and /or create the volume in your cheeks. Everyone is so interested in fillers these days.....and for good reason - they work. "Fillers" is the broad category we use describe treatments that add "products" beneath the skin. They can be used to add or restore volume due to aging or simply to fill in lines and wrinkles Restylane and Juvederm sre the most popular and are made of something called Hyaluronic acid. This naturally occuring building block works by holding on to water within the skin and "plumping". Other fillers such as Radiesse and Sculptra work differently but are equally or more effective in certain areas. Hi everyone,
I just returned from the ASAPS (American Society of Aetsthetic Plastic Surgeons) meeting in Las Vegas. It was a good meeting and re-affirmed my basic approaches to most things. Namely, in order to get a natural result in any cosmetic procedure, whether it is breast augmentation, browlift, facelift or tummy tuck, the plan should be based on things that work. I'm all for innovation and in fact was one of the first in Baltimore to perform laser resurfacing and other cosmetic laser procedures. On the other hand there is a lot of emphasis these days on marketing the "latest and greatest" non-invasive method to do everything. This includes fractional resurfacing, laser liposuction, and laser lipolysis. At this moment in time these modalities work more in theory than on the patient. It seems that you can expect about a 30% response rate. This translates into meaning that the biggest risk of these procedures is a financial one. So if you don't mind the possibility of throwing your money away then there is very little down side.Â

     The Composite Facelift is a technique developed in the early 90's by Dr. Sam Hamra from Dallas, Texas. It is a technique that elevates and lifts the deeper connective layer (SMAS) together with the skin as a single sheet. It is very powerful since it retains better vascularity to the flap. Most facelift techniques today either separate these two layers or simply do not deal with the SMAS very much at all. I learned the technique directly from Dr. Hamra while spending time with him in 1991 as his first fellow. In its purist form it used to raise the flap together with the orbicularis oculi muscle of the lower eyelid. The technique has evolved however and the sub-orbicular dissection has gone a bit deeper so that the two dissection planes are no longer in contnuity but actually overlap a bit. This has reduced post operative swelling and enabled a more vertical-medial lift to the lower lid-cheek unit. This improves the results of the face by filling in the general hollowing that occurs during aging. I now combine this technique with a direct mid-face plication which restores the upper cheek fullness and restores the youthful cheek contour. Most surgeons do not perform this technique because it takes longer and is more technically demanding. I have performed the Composite Facelift on hundreds of patients over the years and after some minor adjustments have found it to be a safe, reliable technique to help patients achieve a wondeerful result. Not everyone needs a Composite Facelift, but together with a browlift I believe it gives the best correction of the aging face which will leave the patient with the most balanced youthful and age-appropriate appearance.

I am introducing this new blog on my site. Rather than just posting articles on the homepage, I will pick out some recent articles which seem interesting. They might appear in the scientific journals or popular press. Whenever possible I will try to include a link to the original publication. The first article has to do with an eyelash growth product. For over a year we have used MD Lash Factor, distributed by Procyte. It is extremely effective. It began as a whim for my wife. When she began to receive unsolicited compliments, we knew there was something going on. She continues to use the product and has been known to find the need to trim her eyelashes when they begin to brush against her sunglasses! We have ordered Latisse, described below.

Latisse, a drug used for eyelash lengthening, has been approved by the Food and Drug Administration. Bimatoprost is the main compound in the product that enables lashes to grow longer and darker. Other eyelash lengthening products have been unsuccessful, such as Jan Marini Skin Research's Age Intervention Eyelash. The FDA claimed the product's containment of bimatoprost caused patients to have swollen eyes; Latisse contains a much smaller amount of bimatoprost. Latisse is manufactured by Allergan and costs $120, available with a physician's prescription. The eyelash lengthening drug, which lasts two to four months, has been deemed a solution to hypotrichosis or lack of hair growth. While physicians say that Latisse could be useful for promoting hair growth in both eyebrows and the scalp, Allergan notes that the company has not explored other uses for the drug. I saw a patient in the office this past week who was so thrilled and outspoken about her reduction that I thought I would share. Before her surgery, not only was she so unhappy about her appearance but she suffered greatly with aching, nagging pain in her shoulders, neck and mid-back. She was nervous about the scars she would trade for but decided to "go for it". Not only has all of her pain gone away but she is able to wear clothing that is much mor flattering and shows off her nice figure. Her sex life is improved with her renewed self confidence and looks. Her scars have healed very well and are "not even the slightest issue". She wants women to know how great the operation is and that they shouldn't give it a second thought.

Eyelid surgery, or blepharoplasty, is one of the easiest cosmetic procedures to have performed. Recovery is very brief and usually you can return to work within a week. Bruising is very mild and it really doesn't hurt. Yet the results are outstanding. Your eyes are the first things people see when they speak to you. They give you character and expression. Gynecomastia is not something you need to live with. Whether you are a teenager, young adult, or later in years, treatment for your "man boobs" is effective and much easier than you think. I have been treating men with gynecomastia (male breast enlargement) for almost twenty years and have seen a huge increase in the number of men coming to the office over the past five years. I suspect it is due to inceased awareness and information obtained through the internet. Fillers, such as Restylane, Perlane, Juvederm, Radiesse and Sculptra, are extremely effective and can last between 6 months and 2 years. Restoring facial volume and improving wrinkles is fast and natural. It is performed privately in the office and ususally there is not any downtime.
