Composite Facelift

      The Composite Facelift is a technique developed in the early 90's by Dr. Sam Hamra from Dallas, Texas. It is a technique that elevates and lifts the deeper connective layer (SMAS) together with the skin as a single sheet. It is very powerful since it retains better vascularity to the flap. Most facelift techniques today either separate these two layers or simply do not deal with the SMAS very much at all. I learned the technique directly from Dr. Hamra while spending time with him in 1991 as his first fellow. In its purist form it used to raise the flap together with the orbicularis oculi muscle of the lower eyelid. The technique has evolved however and the sub-orbicular dissection has gone a bit deeper so that the two dissection planes are no longer in contnuity but actually overlap a bit. This has reduced post operative swelling and enabled a more vertical-medial lift to the lower lid-cheek unit. This improves the results of the face by filling in the general hollowing that occurs during aging. I now combine this technique with a direct mid-face plication which restores the upper cheek fullness and restores the youthful cheek contour. Most surgeons do not perform this technique because it takes longer and is more technically demanding.  I have performed the Composite Facelift on hundreds of patients over the years and after some minor adjustments have found it to be a safe, reliable technique to help patients achieve a wondeerful result. Not everyone needs a Composite Facelift, but together with a browlift I believe it gives the best correction of the aging face which will leave the patient with the most balanced youthful and age-appropriate appearance.
