The Composite Facelift is a technique developed in the early 90's by Dr. Sam Hamra from Dallas, Texas. It is a technique that elevates and lifts the deeper connective layer (SMAS) together with the skin as a single sheet. It is very powerful since it retains better vascularity to the flap. Most facelift techniques today either separate these two layers or simply do not deal with the SMAS very much at all. I learned the technique directly from Dr. Hamra while spending time with him in 1991 as his first fellow. In its purist form it used to raise the flap together with the orbicularis oculi muscle of the lower eyelid. The technique has evolved however and the sub-orbicular dissection has gone a bit deeper so that the two dissection planes are no longer in contnuity but actually overlap a bit.
